Care Coordination Inc. is a community-based organization supporting individuals and families across Maryland in accessing resource navigation, referral support, and eligibility-based case management services. Our mission is to connect underserved populations with the services and supports they need to thrive in their communities.

Community Resource Navigation

CCI helps individuals & families identify & connect with local and statewide resources that support health, housing, transportation, food access, behavioral health, and more. Our team provides personalized guidance to ensure every individual can navigate systems of care with confidence.

Eligibility-Based Services Support

We assist community members in understanding and applying for public benefits such as Medicaid, SNAP, SSI, and other eligibility-based programs. CCI provides step-by-step support to help individuals gather necessary documentation, complete applications, and access the services for which they qualify.

Outreach & Referral Coordination

CCI partners with community organizations, clinics, schools, and public agencies to provide coordinated referrals and outreach. We ensure individuals are linked to services that meet their unique needs through warm handoffs and follow-up support.

Advocacy for Individuals with Complex Needs

Our staff advocates for individuals facing significant barriers to care due to disability, poverty, trauma, or systemic inequities. We work alongside families and caregivers to address gaps in care, reduce service fragmentation, and promote equitable access.

School-Based Advocacy for Children with Disabilities

CCI provides advocacy and navigation support for families of children with disabilities, helping them access school-based services, IEP/504 accommodations, and special education resources. We empower caregivers to advocate effectively within the school system.
